A Web portal for maintaining Personal Health Records

For Patients:

Synergy Personal Health Record is a secure web portal used by patients (and their providers) to create, maintain and communicate their Personal Health Record. In minutes, you can create a Personal Health Record for you. With this portal information you can feel confident that you'll have access to your complete health information anywhere and at any time for any of your healthcare providers. Also a patient can print your Personal Health Record information to take with you and your loved ones when you see your physician/provider and healthcare providers. Also a patient/user can communicate with the provider through the secured Message board

Your complete medical history. Always with you. Secure and Online, Where you and your doctors can always access it. You control it. You decide who and when your record is seen.

For Providers:

Synergy Personal Health Record is used by providers to access a patient’s Personal Health Record. Providers can access their patients PHR two ways. First, when a patient provides their Guest PIN, the physician can access via the Internet to view their Patient Health Report. A provider can look in to the patient record and can be send the information to the patient through the Message board.

Doctor:

Doctor / Provider are a user, who will register with the system.

Administrator:

An administrator is super user and having the complete control over the system.
